Federal regulators have cited a chemical company for a dozen violations at a plant in Nitro where a worker was killed in June.
The U.S. Occupational Safety and Health Administration announced the citations against AC&S Inc. on Thursday.
OSHA said the worker died while sandlblasting. Federal investigators found that the air line for the worker's air hood was hooked up to a nitrogen gas line.
The alleged violations include failing to properly label gas systems and failing to provide training on hazardous chemicals.
OSHA saids all 12 violations are serious. That means the employer knew or should have known about a hazard that could result in death or serious physical harm.
A telephone call to the plant was referred to a marketing firm, which did not immediately return a message.
